The Value and Vision of PCMH

One of the most promising health care delivery models is called Patient-Centered Medical Home (PCMH). PCMH emanates from the Planetree movement to create environments for care that exhibit kindness, care, and respect.

PCMH is about providing healthcare in a more personal and managed way. Primary and preventative care are provided by a care team that is led by a dedicated physician and a nurse practitioner or mid-level provider that oversee all aspects of patient care in a highly individualized fashion.

The care team is augmented by a team of onsite specialists, complimentary medicine, and a robust electronic medical records system. A PCMH may exist within a hospital, a freestanding clinic, or a specialty center.

Designing facilities that support the PCMH delivery model requires collaboration between the medical community and its architectural and engineering partners. Communication is critical from planning and design all the way through construction and operation of the facilities. What follows is a case study featuring two examples of how Kluber Architects + Engineers engaged with their client partners to develop facilities that support the PCMH model for health care facilities.

K4 High Performance Facilities Improvement Plan

Are you prepared for the expected, the unexpected, and the potential for higher performing facilities?

The right facilities maintenance plan provides a critical planning tool and help make informed decisions. With a K4 plan you can set budgets, project priorities, and resource allocation with confidence. It can also serve as a road map for improving the overall performance of your facilities. It all starts will the right perspective and right type of assessment.

At Kluber, our architects and engineers take a different approach to making facilities and mechanical systems more energy efficient. Our K4 High Performance Facilities Improvement Plan is a performance-based maintenance strategy that can guide and inform facility owners throughout the life of their building.

The plan includes informed decisions on potential capital projects, renovation or replacement strategies for everything from HVAC equipment to paint and flooring selections. A personalized maintenance plan can minimize breakdowns, improve energy efficiency, extend operational life, and manage uncertainty for building operations and maintenance. It is a single-source course of action outline that combines health and life safety surveys, space utilization studies, replacement schedules, preventative maintenance plans and capital improvement schedules.
