Project Details

The Lake County Health Department and Community Health Center received a State of Illinois construction grant for 3 million dollars to support the funding effort in the construction of a satellite public health care clinic in Zion, Illinois. Kluber worked with the Lake County Health Department to design this facility to conform to the Patient-Centered Medical Home (PCMH) model for care for this 13,000 square foot treatment facility.
The clinic that now serves the Zion community provides primary care, mental health care, dental care, counseling services, a Women, Infants and Children clinic and community-use multipurpose rooms. The facility has a large, security-controlled waiting lobby with access to private patient registration desks. The medical clinic portion of the building has been designed with four distinct “team rooms” and each team room serves three exam or dental laboratories and one counseling office.
